Separate the bills. Hold a vote on extending the Bush tax cuts for all incomes below $250.000 and then do a separate bill for extending the tax cuts for incomes over $250.000. The Republicans will do anything to stop the first bill including a filibuster in the Senate. Let them do it so that maybe a few more Americans will wake up and see what the GOP's priorities really are.
